Templates and documentation structures for household seed phrase location indexes and continuity records.
What Planning Records Are
Planning records are structured documents that describe where recovery materials are stored, who may access them, and under what conditions — without containing the seed phrase content itself. They serve as the map successors follow during an access event.
Toolkitheatherpath produces these records during consultations. This page explains the templates we use so households understand what they receive and what they can prepare in advance.
Household Location Index
The core document. Each page covers one wallet or backup set:

| Device type | Hardware wallet brand and model |
| Backup medium | Paper card, metal plate, or engraved tag |
| Storage location | Room, building, container type |
| Access method | Key location, combination holder, deposit box number |
| Last verified | Date the location was confirmed intact |
The index deliberately excludes seed word content, PINs, passphrases, or private keys.
Successor Briefing Outline
A companion document assigning roles:
- Primary successor — first person to act when a trigger condition is met
- Secondary successor — acts if primary is unavailable
- Condition triggers — incapacity documentation, death certificate, or voluntary handover letter
- Step sequence — numbered actions in order (locate index, verify condition, access location A, then B)
Condition Trigger Sheet
Defines the events that authorize access. Common triggers include:
- Certified medical incapacity with two physician signatures
- Registered death certificate presented to the bank for deposit box access
- Written handover letter signed by the primary holder
What We Provide vs What You Prepare
During a Household Continuity Session, we complete all three documents on-site. If you prefer to begin independently, download is not available — records are customized per household during the consultation. You may, however, prepare a draft inventory list before your session to shorten the on-site interview.
Record Storage Recommendations
Completed records should be stored separately from seed phrase backups:
- With estate planning documents held by your attorney
- In a known filing location both spouses can access
- Never inside the same container as the seed phrase itself
Annual Updates
Records become outdated when wallets are added, storage locations change, or successors are reassigned. Schedule an Annual Record Review to keep documents current.
